Moves, Adds and Changes (MAC) Forms

Below is a selection of 2degrees Managed Services MAC forms for requesting changes to your existing services.

These editable PDF forms should be completed and sent to the email address provided in the Contact Details section of each form.

Urgent requests will be actioned within 4 hours (subject to availability).

A Simple MAC request is defined as:

  • A MAC request that can be carried out remotely by a Managed Services Engineer and does not require attendance at a customer's site.
  • A request to change the characteristics within an existing service or services for a customer.
  • Up to 5 MACs of the same type within an existing service where the effort to complete the request is no more than 4 hours.
  • A MAC request that does not require a planned outage event or notification.

A Complex MAC request is defined as:

  • A MAC request to change the characteristics of an existing service or services that has an effort of work greater than 4 hours.
  • A MAC request for multiple changes on multiple devices or services.
  • A MAC request where a quote is required to scope the effort required to complete the work. This will be provided within 2 business days of receiving the request from the customer.
  • A MAC request where an approved Purchase Order is required from the customer before the work can commence.

The following are exceptions to the above:

  • Any request that requires a product change, price or contract change to the monthly billing or a solution design change. These types of requests are outside of the scope of the MAC process and should be directed to your Account Manager or Service Delivery Manager.
  • Any urgent Complex MAC request. These will be reviewed by the Managed Services team and handled on a case-by-case basis.
